Child Dedication Questionnaire

A Child Dedication is a moment to honor and celebrate the new life of your child with your Pea Ridge Church family. In doing this practice, we thank God for the children among us, affirm their value as God’s image bearers, and welcome them into our family. We’ve provided a guide to help you mark this special moment. Please complete the basic information below. If for some reason you do not wish to share a grandparent's name, simply leave that box blank. Our pastor will share this information as he introduces your family.

During your child’s dedication, there will be an opportunity for you or Pastor Dan to share more about your child with the congregation. We understand that not everyone is comfortable speaking in front of a group so Pastor Dan will be glad to read your responses if you would like. Fill out the below worksheet and make sure to indicate who will be reading the responses. Be mindful of the length of answers—we will have about 3 minutes to share.
